Outreach Supports - Providing support to access your community, develop your individual life-skills and build independence - all in support of your goals. Recovery Coaching and Support Coordination - Through lived or learnt experience we will assist you to access support, utilise your NDIS plan and support you to live a meaningful life, relevant to your goals. Youth Transition to Adulthood - Through facilitating Rites of Passage camps, we support, recognise and honour teenagers in their transition to adulthood. Transition to Home - We provide support to patients discharged from acute mental health services to find suitable long term accommodation and community supports.
